    Welcome to our office!
    Making an appointment is the critical first step in solving your foot and ankle problem. Being well prepared for your appointment ensures that we have everything needed to provide the best possible care for you. Before your first appointment there are a few special steps to take and some things you need to know so we can best treat your foot and ankle conditions.

    Patient Forms
    Below is a link to our New Patient form. Please print and complete it before your appointment date. Doing this at home will make it easier for you to provide an accurate medical history, as well as a list of all oral and topical medications that you use.  Be sure to identify any allergies that you may have.

    Bring along any foot orthotics (inserts), off the shelf arch supports or other foot devices that you use or have used in the past.

    It is also important that we check your footwear. Please bring 2-3 pairs of shoes that you wear the most including dress, casual, athletic shoes or walking shoes.

    Take Hwy 26 W to Collingwood.

    Continue onto Hume St.

    Turn right onto St. Marie St.

    Destination will be found on the left.

    *the clinic is located 2 houses south of the Giant Tiger*

     

    From Thornbury/Clarksburg area:

    Take Hwy 26 E to Collingwood

    Turn left onto First St/ON-26 E

    Turn right onto St. Marie St (runs parallel with Hurontario St)

    Continue pass the Giant Tiger.

    Destination will be on the Right.

    *the clinic is located 2 houses south of the Giant Tiger.
