What are custom foot orthotics?

Custom foot orthotics are shoe inserts made from a three-dimensional model of your own feet. They are designed to support, cushion or redirect pressure on the foot, based on a written prescription for your condition. Over-the-counter insoles are made in standard sizes and are not built from a model of your feet.

Who might be assessed for orthotics?

People often book an orthotic assessment when they notice:

  • Ongoing pain in the heel, arch or ball of the foot, especially after standing or walking
  • Shoes that wear unevenly
  • Repeated ankle sprains, shin pain or knee pain during activity
  • Foot fatigue that builds over a long shift on your feet

An assessment does not always lead to orthotics. Depending on what we find, the plan may include footwear changes, exercises, other treatment, or a referral.

How the process works

  1. Assessment. We take your health history and examine your feet, ankles and lower limbs. This includes how your joints move, how you stand and how you walk.
  2. Posture scan. A standing body scan records your alignment from head to toe. It is used alongside the hands-on exam, not instead of it.
  3. 3D foot model. We take a three-dimensional scan of each foot in a non-weight-bearing position. Tony checks the scan against your feet before you leave.
  4. Prescription. A prescription is prepared that sets the materials, flexibility and corrections for your orthotics. Your footwear, work, activities and health are part of that decision.
  5. Manufacturing. The orthotics are 3D printed by our lab partner to the prescription.
  6. Fitting. Tony fits the orthotics, explains how to break them in, and talks about suitable footwear.
  7. Follow-up. We offer a follow-up visit about 8 weeks after fitting to check comfort and make adjustments. After that, we recommend check-ups as your situation requires.

Before we begin, we explain what you can reasonably expect from orthotics for your condition and how we will judge progress. Results vary from person to person.

Orthotics for different needs

  • Work. For people on their feet in safety boots at construction sites, plants and warehouses, we prescribe orthotics built for work footwear and long shifts.
  • Sport and recreation. For running, golf, hockey, skiing and other activities, the prescription accounts for the footwear and movements of your sport.
  • Older adults. For age-related changes in the feet, orthotics can be designed with extra cushioning and stability in mind.
  • Children. For flat feet, toe-walking or other developmental concerns, we assess whether orthotics are appropriate and review them as the feet grow.

Many extended health plans cover custom orthotics prescribed and dispensed by a chiropodist. Coverage, limits and paperwork vary by plan, so check with your insurer before your visit. We provide the receipts and all the documentation your plan requests.

FAQ

Who makes custom orthotics at Abbott Foot & Ankle Clinic? Tony Abbott, Chiropodist, assesses, prescribes and dispenses every pair. The orthotics are manufactured by an orthotic lab to his written prescription.

Do I need a doctor's referral? No. You can book directly with a chiropodist in Ontario. Some insurance plans ask for a physician's note, so check your plan.

How long does it take to get used to orthotics? Most people adjust within 2-3 weeks. We give you a written and verbal break-in schedule at your fitting.

Can I move my orthotics between shoes? Yes, as long as the shoes are a similar style and depth. Very different shoes, such as dress shoes and work boots, may need a different design.

How do I care for my orthotics? Wipe them with a damp cloth as needed and let them air dry. If they are full length, take them out of your shoes overnight so they can dry.

How often should my orthotics be checked? We offer a follow-up about 8 weeks after fitting. After that, we suggest a check-up when your symptoms change or as your condition changes.

When might I need new orthotics? Consider a reassessment if the orthotics show significant wear, your symptoms return, your weight changes significantly, or your activity level changes.  Regardless of your situation, we attempt to contact you for an annual review of your orthotics.

Will I need orthotics forever? It depends on your condition. Some people use them for a set period; others use them long term.
